CAHSAH Care Giver Live Scan Registration

CAHSAH has partnered with Capital Live Scan to better help
streamline the live scan process. This system will allow you to register for
your specific facility, fill out your form digitally, and make an appointment
at one of our hundreds of locations

Getting Started

By hitting the red "Get Started" button, the system will ask
you to create an account. You can either continue as a guest or create a full
account. We recommend creating a full account so that you will be able to track
the progress of your appointment by clicking the "Check My Status" button.

What You'll Need

The registration process will ask for the following information:

Facility where you work (Name, Address, Number)

Social Security Number
‍
Driver's License Number
